A doctor treating high blood pressure, diabetes or severe mental illness saw the patient at visits and had little view of what happened at home between them. When a patient on a prescribed drug did not improve, the doctor could not tell whether the drug was failing or had not been taken, and might change a treatment that was working, at a cost to both the patient and the health system PRTX-1. Drug makers counted the same gap as lost sales: non-adherence cost pharmaceutical companies about $250 billion a year in potential revenue PRTX-6. Self-insured employers and health plans paid for chronic conditions such as diabetes and cardiovascular disease, which cost more than $500 billion a year in the United States PRTX-10. Each of these buyers wanted information from the patient's daily life, and each had to decide whether that information was worth paying for.
Proteus Digital Health
Livongo made a glucose check send its own data and billed employers and health plans for each enrolled member, while Proteus asked patients to swallow a sensor, wear a patch and run a phone app, and depended on the drug maker that bought its assets out of bankruptcy for $15 million.
Why Proteus Digital Health lost to Livongo Health
Versus Livongo Health: Livongo made a habit people with diabetes already had send its data on its own, and billed the employer or health plan for each enrolled member PRTX-7 PRTX-9. Proteus added a sensor, a patch and a phone to the patient's day and came to depend on the drug maker whose pill carried the sensor PR2 PR6.

How each step happened
Step 1 of 4 · 2016–17
A sensor pill, a weekly patch and a phone
A habit that sent its own data
Livongo started from something people with diabetes already did several times a day. Glen Tullman, the former Allscripts chief executive, launched it in September 2014 with In Touch, a touchscreen glucose meter with its own cellular connection that sent each reading automatically, needed no smartphone and required no test strips to be ordered PRTX-7. Certified diabetes educators sat behind the device, and by 2016 the program included unlimited strips at no extra charge PRTX-7 PRTX-8. Tullman says a member could reach a person within 90 seconds at any hour PRTX-10. The patient kept checking blood sugar, and the data reached the care team on its own.
Three new things to manage
Proteus added tasks. Founded in 2001 in Redwood City, it raised $487 million over 11 rounds from backers including Novartis Venture Fund and Kaiser Permanente Ventures and was valued at $1.5 billion PRTX-5. Its sensor system, Proteus Discover, first used by a health system in January 2016, needed a specialty pharmacy to put an ingestible sensor into each patient's medication and a patch on the arm to record when the pill was swallowed PRTX-1. Abilify MyCite, approved for Otsuka in November 2017, sent the signal from pill to patch to a phone app and a web portal for clinicians and caregivers PR1 PR2. Its label told clinicians to make sure the patient was "capable and willing to use smartphones and apps" and to have the patch changed weekly, and it reported rashes at the patch site in 12.4% of patients PR2. The FDA noted that "detection may be delayed or may not occur" PR1. By late 2019, sources said patients had not widely adopted the digital pill PRTX-4.
Rivals Livongo's In Touch meter sent glucose readings over its own cellular link and came with unlimited strips and a virtual care team of diabetes educators PRTX-7 PRTX-8.
Step 2 of 4 · 2018
Dependent on the drug maker
Livongo sold to the organizations that paid for a patient's care. Its clients were self-insured employers, health plans, government entities and labor unions, on one- to three-year subscriptions charged per participant per month, so fees depended on how many members enrolled PRTX-9. Lowe's offered the program to employees with diabetes in North Carolina, South Carolina and Alabama, then opened it to employees and dependents nationwide in March 2016 PRTX-14. By then Livongo had signed more than 50 large customers, including Office Depot and SC Johnson, and payers including Humana and Cigna, and its users had doubled to 10,000 in a year PRTX-8. Vasudev Bailey of Artis Ventures, who did not invest in Proteus, credits Livongo's success partly to settling early who would pay PRTX-6.
Proteus came to depend on a drug maker. In April 2016 it raised a $50 million Series H, taking total funding above $220 million, and chief executive Andrew Thompson described a staged approach to building the market PRTX-2. In October 2018 Otsuka expanded the collaboration and made $88 million in equity and other payments to Proteus under an agreement covering digital medicines for five years PR6. The health-system route had no single payer: at Barton Health, the chief executive said reimbursement would vary by carrier PRTX-1. Abilify MyCite cost upwards of $1,600 a month, against $500 to $800 for generic Abilify PRTX-6.
Rivals Livongo billed employers, health plans and unions each month for every enrolled member, and Lowe's widened its program from three states to the whole country in March 2016 PRTX-9 PRTX-14.
Step 3 of 4 · 2016–19
From disease to disease
Livongo grew by selling more conditions to the clients it already had. By 2019 it sold Livongo for Diabetes, Livongo for Hypertension, Livongo for Prediabetes and Weight Management, launched in April 2018, and Livongo for Behavioral Health by myStrength, launched in February 2019 PRTX-9. Client retention was 96%, and the dollar-based net expansion rate was 113.8% at the end of 2018 PRTX-9. In May 2019 a Lilly-funded study at three employers found that medical spending among enrolled members fell 21.9% after one year, or $88 per member per month, against an average employer cost of $68 PRTX-11.
Proteus moved its sensor from disease to disease. Its 2016 pilot in uncontrolled hypertension and type 2 diabetes had 85% of 72 patients on Discover reaching their blood pressure target by week four, against 33% of 24 on standard care PRTX-3. The commercial work then went to severe mental illness with Otsuka PR6, whose label stated that its ability "to improve patient compliance or modify aripiprazole dosage has not been established" PR2. By January 2019 Proteus had begun moving into oncology, and by December it pointed to studies in hepatitis C, tuberculosis and cardiometabolic syndrome PRTX-5 PRTX-4. Everett Crosland, a digital therapeutics executive, says the partnership strategy did not push early enough toward the randomized trials and health-economics studies that drive coverage PRTX-5.
Rivals Livongo added hypertension, prediabetes and behavioral health programs for its existing clients and published employer cost savings in 2019 PRTX-9 PRTX-11.
Step 4 of 4 · 2019–20
Bankrupt, and sold to its partner for $15 million
Livongo's evidence and wider offer carried it into mainstream employers. Revenue grew from $30.9 million in 2017 to $68.4 million in 2018 PRTX-9 and $170.2 million in 2019, up 149% PRTX-12. At the end of 2019 it had 222,700 Livongo for Diabetes members, up 96% in a year, and reached more than 30% of Fortune 500 companies PRTX-12. On August 5, 2020, Teladoc Health announced a merger with Livongo in an exchange representing a value of $18.5 billion PRTX-13.
Proteus reached the same years leaning on its partner. In late 2019 a $100 million funding round failed to close: Otsuka was not ready to commit more, and other investors would not proceed without supporting data PRTX-4. Proteus filed for Chapter 11 on June 15, 2020 PR3. Its banker, Raymond James, contacted 240 potential buyers, who passed because Proteus was burning $2 million to $2.5 million a month with no clear date for profit PR3. Otsuka, one of its main customers and the primary licensee of much of its intellectual property, made a $15 million offer that included certain liabilities PR3. Equity holders including Novartis objected that the sale would leave investors with $500 million in equity "holding the bag" PR3. On August 25, 2020, Otsuka announced that it had purchased the assets, so the drug maker that had paid $88 million into the partnership in 2018 ended up owning the sensor PR5 PR6.
Rivals Livongo reached $170.2 million of revenue in 2019, and Teladoc Health agreed to merge with it in August 2020 in an exchange representing a value of $18.5 billion PRTX-12 PRTX-13.
