Science 37

Science 37 built a separate company to run clinical trials away from research sites and sold much of it through CROs, while IQVIA added home visits and telehealth to the trials it already ran; Science 37 was sold to eMed in 2024 in a deal valued at about $38 million.

Why Science 37 lost to IQVIA

  1. Tech-Enabled Services

Versus IQVIA: IQVIA added telehealth, home nursing and virtual site management to the trials it already managed, so sponsors could get decentralized options from their existing CRO SCIX-3 SCIX-8. Science 37 sold decentralized trials as a separate service, much of it through CROs, and carried each study's staffing and cancellation risk on a far smaller base SC2 SC1.

Barriers to participationEntrenched systems

A drug company testing a new treatment in 2014 hired a contract research organization (CRO) to run the trial. The CRO contracted research sites to enroll patients and sent monitors to each site's premises to check quality and compliance SC2. Patients had to travel to the site for every visit in the study schedule, and only about 8% of patients were ever approached to join a trial, because most did not live near a participating site SC2 SC1. Each site ran its own operating procedures, and CROs had built their processes around those sites, which made a shift to visits at home or online a change in how the whole trial was run SC2 SC1.

How each step happened

Step 1 of 4 · 2014–21

Patients recruited without a site

Two answers to the site problem

IQVIA attacked slow enrollment by making the existing trial model smarter. In May 2016 Quintiles, a CRO, agreed to merge with IMS Health, a healthcare data company, with the stated aim of improving trial design, recruitment and execution using IMS's patient information SCIX-9. By 2021 the combined company, IQVIA, held more than 1.2 billion non-identified patient records and used analytics for site identification and patient recruitment SCIX-4. Sponsors could keep their CRO and their procedures and still get better site choices and patient lists.

Science 37, founded in 2014 by Noah Craft and Belinda Tan, who served as its chief executive and chief medical officer, attacked the site itself SC1. It recruited patients independent of any site, often through media or referrals from patient groups, providers and pharmacies, and served them at home through its own telemedicine investigators, mobile nurses and remote coordinators working on one technology platform SC1. By early 2022 it reported more than 100 trials executed and over 500,000 patients engaged SC1. Sponsors bought the work of running the trial along with the software, which spared them assembling those pieces SC1. The same choice put Science 37's own staff and money behind each study: some trials required large upfront spending on patient recruitment that sponsors did not always repay SC1. David Coman, formerly IQVIA's chief marketing officer, became chief executive in November 2019, and the company settled with both founders in June 2020 SC1.

Rivals IQVIA combined Quintiles' trial operations with IMS Health's data in 2016 and by 2021 used more than 1.2 billion patient records to choose sites and find patients SCIX-9 SCIX-4.

Step 2 of 4 · 2020–22

A virtual site inside CRO-run trials

Many of IQVIA's decentralized trials launched in 2020, as the pandemic disrupted studies, inside trials it already managed. It offered telehealth, virtual site management, home health nursing and direct-to-patient engagement, and let sponsors choose fully virtual or hybrid designs SCIX-3. By March 2021 more than 60 registration trials in over 40 countries used these services, IQVIA had enrolled more than 150,000 subjects since starting the work, and it ran Johnson & Johnson's late-stage COVID-19 vaccine test virtually SCIX-3 SCIX-2. By October 2021, 30% of IQVIA's studies used at least one decentralized component, and 89 ran fully on its platform for 34 sponsors SCIX-8. A sponsor could add home visits without hiring a second company.

Science 37 had positioned itself as that second company. It offered full decentralized trials, served as a "Metasite" handling part of the patients in a trial run through traditional sites, or licensed its technology alone SC2. It chose to grow through the CROs: "Science 37 views CROs as partners and a sales channel" SC2. PPD, a CRO that was also a beneficial owner of Science 37, supplied 22.9% of its 2021 revenue SC1. In May 2022 CEO David Coman credited CRO partnerships for a record pipeline SCIX-7. Some CROs already called Science 37 a competitor SC2. In April 2022 PPD, by then owned by Thermo Fisher, folded Matrix Clinical Trials' mobile research sites and virtual investigator network into its own hybrid and decentralized portfolio, and in October 2022 it launched a network of DCT-trained sites with the stated aim of becoming customers' preferred DCT partner SCIX-14 SCIX-13 SC2. Science 37's revenue from PPD fell from $13.7 million in 2021 to $6.9 million in 2022 SC2.

Rivals IQVIA offered decentralized and hybrid designs inside its own full-service trials, used in 30% of its studies by October 2021 SCIX-8 SCIX-3.

End-to-End Workflow

Step 3 of 4 · 2022–23

A backlog sponsors could cancel

For IQVIA, decentralized tools lowered the labor in work it had already won. CEO Ari Bousbib told investors in October 2021 that the more technology IQVIA deployed, the fewer people it needed and the more margin it made, while customers would pay no more than before SCIX-8. IQVIA could apply those savings across a contracted R&D Solutions backlog of $24.8 billion at the end of 2021, which rose to $27.2 billion a year later on record bookings, with a book-to-bill ratio of 1.36 including reimbursed expenses SCIX-5 SCIX-12.

Science 37 carried the same project risk on a far smaller base. Most of its project customers could terminate without cause on 30 to 90 days' notice, and its cancellation rate for project-based Phase I through IV trials rose from 9.2% in 2021 to 36.9% in 2022 SC2. Contracts paid for fees earned, wind-down work and non-cancellable expenditures, but gave Science 37 no right to the full revenue in its backlog SC2 SC1. Net bookings fell 51.7% to $79.2 million in 2022, which the company put down to longer sales cycles, two material COVID cancellations and one down-scope SC2. Revenue rose 17.7% to $70.1 million while cost of revenue rose 28.0% to $54.3 million SC2. In November 2022 Science 37 cut 81 employees, about 15% of its workforce SC2.

Rivals IQVIA reported record bookings and an R&D Solutions backlog of $27.2 billion at the end of 2022 SCIX-12.

Operational Excellence

Step 4 of 4 · 2021–24

Sold to eMed for $5.75 a share

IQVIA's base kept growing. In 2021 it held about 56 petabytes of data from roughly 150,000 data suppliers and employed about 79,000 people in more than 100 countries SCIX-4. Its R&D Solutions backlog reached $32.7 billion at the end of 2025 SCIX-10.

Science 37 had gone public on an ambitious growth forecast. Its October 2021 SPAC merger valued it at more than $1 billion, about 5.8 times the $182 million of revenue projected for 2023, and raised $235 million SCIX-6 SC1. Its 2022 revenue of $70.1 million fell short of the $100 million forecast at the merger, and revenue for the first nine months of 2023 fell 18.2% to $44.3 million SCIX-6. Cash dropped from $108.1 million at the end of 2022 to $56.4 million by September 30, 2023 SCIX-11. In January 2024 eMed, a telehealth and diagnostics company, agreed to pay $5.75 a share in a deal valued at about $38 million SCIX-1. About 77.8% of shares were tendered, and eMed accepted them on March 12, 2024 SC4. The service model from step 1 had brought patients into trials, but every study it won still depended on Science 37's own staff and on sponsors keeping the trial alive.

Rivals IQVIA employed about 79,000 people in more than 100 countries and held more than 1.2 billion patient records in 2021, while Science 37's cash fell to $56.4 million by September 2023 SCIX-4 SCIX-11.
